Tetracosactide, also known as cosyntropin, is a synthetic polypeptide consisting of the first 24 amino acids of human adrenocorticotropic hormone (ACTH).
It acts by stimulating the adrenal cortex to synthesize and release cortisol, adrenal androgens and other corticosteroids.
Its primary clinical use is for the adrenocorticotropin stimulation test (ACTH stimulation test). This diagnostic test assesses whether the adrenal glands can produce sufficient cortisol, to screen and confirm adrenal insufficiency, including primary adrenal failure and secondary adrenal hypofunction.
When administered parenterally, tetracosactide triggers a rapid rise in serum cortisol levels in patients with intact adrenal function. If the adrenal response is inadequate, it indicates adrenal impairment.
It is mainly a diagnostic agent, not a long-term therapeutic hormone replacement drug.
