Pharmacological effect
Target: PTH1 receptor (PTH1R), biased activation of RG conformation, promoting bone formation > bone resorption. Chinese Medical Association...
Selective activation of osteoblasts promotes new bone formation (assimilation of bone-like drugs), while the degree of osteoclastic activation is weaker than that of teriparatide, and the incidence of hypercalcemia is lower. It can simultaneously enhance the bone density and bone strength of trabecular bone and cortical bone.
2. For the kidneys: It temporarily promotes calcium reabsorption in the renal tubules and increases urinary phosphorus excretion.
3. Bone remodeling: With intermittent daily administration, bone formation significantly exceeds bone resorption, resulting in a net increase in bone mass. A sustained high concentration is the opposite.
The key difference in bitriparatide:
• Abaloparatide: An analogue of PTHrP (1-34), preferring the RG conformation, with weaker bone resorption stimulation and less hypercalcium; The upper limit of the treatment course is 18 months
• Teriparatide: PTH (1-34), both R0 and RG are activated, with an upper treatment limit of 24 months
